Claiming health-care expenses

If you have visited a health-care professional (doctor, pharmacist, therapist or hospital), the invoice may be sent to you rather than directly to ONVZ. If the expenses are covered by your basic health insurance policy or supplementary health insurance policy, then you can claim these expenses from us.

Claims by letter

You can download a claim form, fill it in and send it to us, along with your original invoices. ONVZ pays the medical expenses to you.

Electronic claims

You can also submit your claim electronically through MijnONVZ. You can save the claim entered for your own administration. To submit the claim, please print out the PDF file and send it to us, along with your original invoices.

How to claim in 3 steps

  1. Fill in the claim form as completely as possible.
  2. Include the original invoices (!) with the claim form. We will not process copies and (formal) reminders.
  3. Please send your claim to:

Handling claims

You can check the status of your claims on MijnONVZ. If you want your invoices to be handled as quickly as possible, we advise you not to save them up, but to send the invoices through as soon as you have received them.

We aim to pay claims to you within 3 to 5 working days.

If you have sent us a claim form, you will receive a new claim form when we send you the payment notification.
