Check whether you need to apply for a basic health-care plan 

If you will be working in the Netherlands while living in the Netherlands or abroad, you have to apply for a basic health-care plan in the Netherlands with a Dutch health insurer.

Coming to the Netherlands as a highly skilled migrant

As a highly skilled migrant, you are eligible for Dutch health insurance if you are coming to work in the Netherlands.

How to apply for a basic health-care plan with ONVZ

Do you wish to apply for a basic health-care plan with ONVZ? Please send the information and documents below along with your application. Based on the information you provide, we will determine the commencement date of your ONVZ basic health-care plan.

  • Your Citizen Service Number (BSN). You will get this number after registering with a municipality in the Netherlands or you may get it from the Dutch tax authorities.
  • If we need additional documents, we will contact you.

Do you live in an EU/EEA or treaty country?

And do you need health care in your country of residence? With an E106/S1 form:

  • You will be able to prove you have health insurance in the Netherlands.
  • You register with a health insurer in your country of residence for health insurance. Usually, you do not pay extra premium for this health insurance.
  • We will reimburse medical expenses incurred in your country of residence according to the rules of that country's health-care system.

Do you have family?

If you have family members that do not live or work in the Netherlands, they cannot apply for health insurance in the Netherlands. If any of your family members live or work in the Netherlands, you can apply for a basic health-care plan for those family members. We will determine whether they are eligible for Dutch health insurance. We may need extra information from you to do so.
