Add a person to the policy

The primary insured person under a health-care plan can have one or more persons added to the policy. Read on to find out how this works in different situations.

The person being added is already a customer of ONVZ

  • The primary insured person and the person who wants to be added to the policy both need to send an email to polisteam@onvz.nl.
  • In both emails, please state the policy number to which the person should be added.
  • In both emails, please state the current customer or policy number of the person who wants to be added.
  • The primary insured person will receive a new policy document within 10 days of receipt of both emails.

Please note: if the person being added is under 18, the current primary insured person for the person under 18 must send the email on their behalf.  

The person being added is not a customer of ONVZ

  • If the person being added is aged 18 or older, they can submit a request at aanvragen.onvz.nl.  
      • The primary insured person and the person who wants to be added to the policy both need to send an email to polisteam@onvz.nl.
      • In both emails, please state the policy number to which the person should be added.
      • In both emails, please state the current customer or policy number of the person who wants to be added.
      • If the person being added is under 18 years old, the primary insured person must submit a request together with the person who wants to be added at aanvragen.onvz.nl.

    If you would like to add a baby to the policy, please call us within 4 months of the birth on +31 (0)30-6396222.
