Application form medicines

For some medicines, you must ask ONVZ for prior authorization. These are: 

1. Medicines that do not have a marketing authorization in the Netherlands
2. Some medicines (Epoprostenol, Iloprost for inhalation, Treprostinil, Odevixibat, Contraceptives)
3. Tapering of antidepressants
4. Passed-on preparations that are not included in the basic health insurance
5. Add-on medicines in the case of off-label use
